Personal Record Review: Care-Related Employment Interruption Corrected
A resident’s unchanged employment dates receive a corrected interpretation and completed downstream review.

Request and agreed facts
Nadia authorized this first-name account. Her record correctly showed a nine-month employment gap in 2023. A provider summary classified it as a voluntary career interruption. Nadia supplied evidence that she had left work to care for her dying parent. She wrote: “The dates are right. The explanation is not. I keep being offered a return-to-work course for a decision I did not make in the way the record suggests.”
How the classification travelled
The original code distinguished absence from dismissal for payroll administration. A career-planning service later interpreted the same code as a preference for reduced employment continuity. That derived field affected access to an advanced training pathway. The archive summary displayed the interpretation without its original limited purpose. Each system had preserved the dates while the meaning changed between them.
Decision and completion
Resident Services replaced the active classification with care-related employment interruption and preserved Nadia’s account separately from the verified dates. The career-planning team removed the reduced-continuity inference, reassessed the application and restored eligibility for the advanced pathway. Nadia accepted a place. The training administrator confirmed the new classification and removed the earlier recommendation from its active profile. All three registered consumers acknowledged the corrected version before the case was closed.
Control improvement
The original payroll event remains in the source history with a supersession notice. Care-related correspondence was inspected by the review officer and was not distributed to training or Housing. Governance required future summaries to identify the purpose of an administrative code before reusing it as a personal preference. Nadia received the revised record and decision notices. The correction changed the service outcome; it did not require her to dispute the accurate dates.
